Summary:

The LISW Therapist is responsible for providing therapy services to patients and their families as part of a multidisciplinary team. Practices in both inpatient and outpatient settings. Demonstrates ability to provide diagnostic impression. Develops, implements, reviews treatment plans and reviews. Demonstrates ability to provide and update clinical assessments. Communicates verbally and in writing with physicians and other providers. Completes psychosocial assessments. Facilitates individual, family, and group therapy. Provides case management, discharge planning while documenting within required time frames. Ability to effectively assess, plan, and implement therapeutic services in a multidisciplinary team setting. Knowledgeable regarding all populations with background in medical social work.

Responsibilities:

  • Demonstrates knowledge and skills necessary to provide services based on the physical, psychosocial, educational, safety, and other related criteria appropriate population in his/her assigned area
  • Maintains required core competencies.
  • Complies with set Policies and Procedures (i.e. name tag, dress code, parking, smoking, etc.)
  • Responds to crisis situations per policy and procedure.
  • Knowledge of current DSM as evidence by appropriate diagnostic impressions and symptoms of mental illness.
  • Discuss with individual patients their responses to events and interactions between individual appointments/ groups.
  • Completes the Initial Screening/Assessment of patient prior to or at admission.
  • Completes psychosocial assessments or update within 3 treatment days of admission.
  • Develops Treatment plans within three (3) treatment days of admission and participates as a team member in the Interdisciplinary Treatment Planning Conference.
  • Completes documentation of medical records as per policy and procedure 100% of the time.
  • Collaborates with the psychiatrist on the patient’s overall treatment.
  • Implements and reviews Treatment Plans with patients.
  • Coordinates treatment team meetings for assigned patients as evidence by chart reviews, observation, and feedback from the team members.
  •  Documents Case conference/team meetings for problematic cases.
  • Documents patients’ group participation, individual and/or family therapy on day of occurrence in medical record.
  • Evaluates data from all sources and recommends referrals as needed.
  • Assesses patients and families for possibility of family therapy.
  • Develops and ensures implementation of a family education plan.
  • Develops lectures and special focus topics for program enhancement as requested.
  • Assists the supervisor with development of educational materials for the patient and family.
  • Assists in formulating Discharge Plan.
  • Understands the process of Advanced Directives and acts upon as per patient’s best interest.
  • Ensures that patient rights and responsibilities are enforced at all times.
  • Provides therapeutic services to patients and their families and groups of patients in relation to:
  • Functional impairments related to their illness and behaviors resulting from psychodynamics’ of their illness.
  • Emotional problems between family and patients in coping with anxieties, stress precipitated by illness and hospitalization
  • Psychoeducation about their illness and aids in coping and improving their quality of life.
  • Provides group, individual and family therapy to patients and family members.
  • Exhibits knowledge of community resources in assisting patients to connect with the community and to receive support from community agencies.
  • Coordinate and ensure delivery of service for assigned patient as evidence by chart reviews, observation, and feedback from physicians
  • Completes patient charge sheets or billing daily.
  • Participates in public relations, develops referral sources and provides community education
  • Participates in behavioral health on-call team.
  • Receives telephone inquiries, provides follow-up on inquiries and assists in assessments as needed.
  • Participates in patient admissions, intakes, and assessments as needed.
  • Understands and has ability to complete mental status exam as well as suicide assessment
  • Understands and utilizes confidentiality related to patients and staff.
  • Performs case management duties as assigned by supervisor.
  • Orients new staff members to therapist role within Behavioral Health Program.
  • Demonstrates skills in identification of abuse situations and follows the process of reporting as per policy and procedure.
  • Provides education training for other staff as it relates to mental status exam, diagnosis, and other topics of interest.
  • Pursues continuing age-related competency per patient population
  • Effectively utilizes knowledge of age-appropriate developmental needs and abilities in the assessment, treatment and education of patients and families.
  •  Other duties as assigned

Requirements:

Education

  • Master’s Degree in Social Work or Psychology with Independent Licensure.

Experience

  • Greater than three years of experience as LISW or LPCC.
  • Demonstrated ability to work effectively with patients, families, physicians, and administration.
  • Demonstrated ability to work well under pressure; to deal with multiple and sometimes conflicting priorities; and to utilize critical thinking skills.
  • Proficiency in computer skills related to electronic health record and Word processing. 
  • Performs related responsibilities as assigned.

Licenses, Registrations, or Certifications

  • Licensure: NM LISW, LCSW, or LPCC
  • Current American Heart – Basic Life Support (BLS) card.
