Summary:

The Coordinator Diabetes Education Lead is responsible for the delivery of patient care education through the process of assessment, diagnosis, planning, implementation, and evaluation. In addition, the Diabetes Coordinator works closely with the Medical Director and oversees the planning, implementation, and evaluation of the Diabetes Self Management Education (DSME) entity. Coordinates diabetes education with other disciplines and collaborates to provide quality patient education. Assists in the follow-up with patients. Serves as a resource and responds to Nursing Service regarding diabetes and related issues affecting nursing activities, and provides updates of diabetes related information as it occurs. Participates in community-wide education.

Requirements:

  • Associate's Degree in Nursing
