Summary:

The Specialty Product Line Coordinator is responsible for the population specific Center of Excellence program design and delivery. Works in collaboration with the physician specialists and hospital based physicians that support the Centers of Excellence. Will maintain a link with the ACNO and Clinical Directors over the staff and operational aspects of the designated hospital unit(s) where this patient population is centered. Strong patient / family coaching interactions including teaching and emotional support, which is expanded to community targeted population teaching opportunities periodically throughout the year. Is also responsible for reporting data from the clinical, financial, satisfaction and outcome perspectives.

Responsible for the overall administration and operations of the TMF Center of Excellence specific to your specialty service / product line and patient population; plus collaboration with other service line program leaders for mutual development and sharing of program process and design.

Leads the Center of Excellence's implementation and operational team meetings. Ensures the timeline and key deliverables of the implementation plan are achieved.

Works in collaboration with marketing and outreach to conduct community education seminars in targeted outreach areas to increase awareness and grow market share. Assists with physician based community education seminars as needed.

Conduct education sessions for the Center of Excellence's patients / family to ensure that all patients / families receive appropriate education regarding the procedure / diagnosis, including preparation for the hospitalization, what to expect during the hospitalization and following transition to home, skilled or rehabilitation services. Works with rehabilitation and other services to ensure a comprehensive patient education and preparation is achieved and maintained on an ongoing basis.

Coordinates the episode of care for all surgery and medical patients including coordination with physician offices to ensure timeliness of diagnostic services and preoperative work ups are complete, follows all patients and coordinates patient care during the acute hospitalization and post discharge follow-up to facilitate a seamless delivery of care process.

Requirements:

  • Current TX RN license, must be in good standing with Board of Nurse Examiners.
  • BCLS
  • Bachelor's degree is preferred.
  • Must be licensed as Registered Nurse with the State of Texas
  • CPR Certification with the American Heart Association or American Red Cross
  • 3 years’ experience as a Registered Nurse in a clinical setting
  • Proficient in Windows, Word and Excel
