Summary:

This position will be expected to proactively contact, schedule and complete Medicare Annual Wellness visits for qualifying patients in the clinic setting. Will work independently, but collaboratively, with the providers and clinical teams at multiple clinic locations to complete patient clinical visits in an efficient manner, follow protocols and standing orders including completion of preventative services and complete appropriate EHR documentation after patient visits. 

Requirements:

• Current Texas State RN License
• CPR (American Heart Association)
• Graduate of an accredited school of nursing.
• Able to assess, plan, implement and evaluate nursing care
• One-year experience in nursing preferred.
