The Lung Nodule Clinical Patient Navigator serves as a single point of contact for physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within CHRISTUS St. Michael Health System and in the community. This position is an advocate for the patient in their dealings with the healthcare community for access to early screening and annual or short term follow-up imaging/testing. This position is responsible for the tracking and reporting of patients enrolled in the program and the generation of periodic reports.

1. Assist patients and physicians in understanding the parameters and benefits of the lung nodule program and how to participate. 2. Develop patient education programs and tools as needed. 3. Serve as an essential link between patients and all other care providers. Follow patients throughout their care continuum. 4. Identifies patients outside the program who are in need of tracking and testing for lung nodules using tools available to track patients for needed follow-ups. 5. Ensures that patients remain informed of the appropriate diagnostic evaluations and/or follow-up plan based on the findings of their initial and annual screening exams and ensures that "the loop is closed," including all additional referrals 6. Records patient data in tracking tools and registry in a timely manner to assure deadlines are met. 7. Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes. 8. Deliver report card to Administration and Cancer Committee and other groups which demonstrate outcomes and performance improvement activities. 9. Adhere to established productivity benchmarks. 10. Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, social services staff, and radiation oncology staff, etc.). 11. Perform all other duties as assigned.

  • Requires excellent leadership, organizational, written, and verbal communication and excellent interpersonal skill.
  • Must be able to work in a self-directed environment, with an ability to work with and lead teams
  • Excellent presentation skills; teaching experience preferred.
  • Ability to implement professional and community based education program.
  • Computer literate is a must; Microsoft Office competency required.
  • Basic Computer Knowledge
  • Ability to communicate effectively, both verbally and in writing.
  • More than five years of nursing
  • Experience in case management or oncolgy or radiology is a plus
  • High school diploma or equivalent.
  • Active Texas Licensed RN, BSN preferred
