We are seeking a Licensed Clinical Social Worker (LCSW) or a Licensed Independent Social Worker Supervisor to oversee and provide professional and comprehensive services such as behavioral health assessment, care management and interventions for patients within an integrated primary care setting. The LCSW or LISW supervisor will also validate patient rating scales and track progress toward treatment goals. Team work and collaboration will be paramount for the LCSW or LISW supervisor, to ensure a model of care that truly provides wraparound treatment ranging from the PCP to the Psychiatric Consultant. Additionally this LCSW or LISW supervisor will revise treatment plans when patients are not progressing, provide brief psychosocial interventions as needed and maintain a registry of patient progress. This position is fulltime M-F, 8-5p.
