  • Coordinates programs that assist patients in utilizing community resources. Gives guidance and support to those patients as they endeavor to overcome social, financial, psychological, and legal barrier to health.
  • Assist patients obtaining medication through the Prescription Assistance Program. This includes but is not limited to:
  • Using computer program for RX Assist to obtain client medications
  • Complete and renew medication applicant forms
  • Catalog and properly store medications upon receipt
  • Notify patients regarding application status, renewals, and medication order arrival
  • Keep and maintain current logs relating to medication, payment, patient information
  • Keep Health Care Provider(s) abreast of medication status for patients and changes with program
  • Coordinate services provided by community agencies. Example of these agencies:
  • The Rose Mobile Mammography, MD Anderson Mobile Mammography, St. Vincent de Paul
  • Give information and assist the patient and their family to access outreach social, medical,
  • Legal, or psychological services as directed by the Provider(s)
  • Follow up with patients to be sure services being sought have been obtained, keeping the
  • Provider(s) informed about the progress
  • Facilitate making appointments as needed for these community agencies/programs
  • Work closely with CHRISTUS Associates as part of the health care team serving the patient
  • Data and Record maintenance REPORTING on all programs being used in this setting
  • Maintain strict confidentiality. Follow CHRISTUS Physician Group policies.


  • Medical Assitant and/or Medical Administrative Assistant preferred. Basic knowledge of clinical/social service skills. Concise and clear language(s), customer service, organized, problem solve, good judgement. Good communication skills and medical background.
  • Bilingual(spanish/english) read,write,speak
  • Network and collaborate with social service programs, ability to access community programs and support services. Use of Computers (Electronic Health Records)
  • HS Diploma, Medical Assistant or Medical Administrative Assistant (Degree/Certified)
