Summary:

The Clinical Pharmacist shall provide the necessary pharmaceutical services needed to facilitate the physician in the treatment of the patient. Such services shall include medication order review, monitoring drug regimens including food/drug and adverse interactions, consulting with providers, nursing staff and patient/family concerning drug therapy, preparation of sterile admixtures, preparation of cytotoxic agents, and providing assistance during “Code Blue” codes. The Clinical Pharmacist will participate in daily interdisciplinary rounding on their assigned unit or where necessary to facilitate pharmacy/nursing/physician collaboration. Clinical Pharmacists will participate in clinical and operational meetings (e.g. PNT, Pharmacy HI, COMT, JOCs) as designated by Clinical Pharmacy Manager. To augment the hospital’s Medication Reconcilliation process, the clinical pharmacist will reconcile patient’s home meds, as required (i.e., direct admits, corrections/clarifications). As a member of the Antibiotic Stewardship Team, the clinical pharmacist will provide antibiotic stewardship services and be responsible for monitoring and coordinating antimicrobial usage in the hospital. The clinical pharmacist will monitor and address therapeutic duplications on the patient eMAR.

In addition, the pharmacist shall be responsible for supporting cost containment programs (i.e., Formulary, ASP) Providing services to patients of all ages at a high competency level.

Requirements:

Education/Skills:

Training in clinical pharmacy, IV compounding & hospital pharmacy perferred.

Training and certification in Antibiotic Stewardship (i.e., SIDP) preferred

Board of Pharmaceutical Specialty certification preferred

Experience:

2 years pharmacy practice preferred. At least one year hospital pharmacy experience desire

License, registrations, or Certifications

2 years pharmacy practice preferred. At least one year hospital pharmacy experience desired.

BLS or ACLS required

Antimicrobial stewardship certification (Active or may be completed within 48 months of hire)

 PMP registration required
