  • This position is responsible for assisting the Care Management Director with supervision, training of staff and leadership of the Care Management Department, as directed by the Care Management Director. Assisting with oversight of Case Managers and Social workers who are providing coordination of care for patients and their support systems.
  • Collaborates with physicians, nursing and all disciplines of care is crucial in determining not only appropriateness of care, but also the continuum of care, discharge planning and post discharge transfer. Must have a working knowledge of resource contacts for continued care.
  • This position is also responsible for auditing UR, transition planning, authorizations, department processes and data abstraction of the medical record for appropriateness and timeliness. Monitoring system LOS & HCAHPS scores. Assisting with implementing improvement processes.
  • Demonstrates mastery and competence in computer skills.
  • Demonstrates knowledge of disease process, normal vs abnormal clinical findings and evidenced based practice.
  • Demonstrate knowledge of regulatory agencies and how to apply.
  • Demonstrates excellent communication skills and quality management skills. Participates as a member of the interdisciplinary healthcare team.
  • Graduate of an accredited school of professional nursing. Minimum requirement is a Bachelor's degree (BSN); Master's (MSN) is preferred.
  • Must be licensed as Registered Nurse with the State of Texas.
  • Must obtain Case Manager Certification within 2 years
  • Three or more years of strong clinical experience in clinical practice area to which assigned.
  • Must have current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.
