Responsible for the delivery and coordination of Diabetes patient education. Collaborates with the interdisciplinary team and leadership to optimize care of the diabetic patient population. Serves as a mentor/educator to clinical associates, patients/family members and the general public.
Responsible for supporting the DSME Coordinator in planning/implementing all aspects of the Diabetic Self-Management Education Program (DSME) in compliance with the ADA DSME Recognition Program.
Coordinates and teaches DSME Outpatient classes for patients referred to the program.
Maintains referral patient records and DSME documentation as required.
Assesses community needs and current evidence based practice to develop/expand diabetes education programs.
Participates in community programs related to diabetes as a presenter and/or resource.
Provides/coordinates diabetic inpatient education based on physician/nurse referrals with required documentation.
Serves as an educator, mentor, and resource for nursing associates who provide direct care to patients diagnosed with diabetes.
BSN in Nursing required.
Basic computer knowledge/skills; ability to communicate effectively both verbally and in written form; general knowledge of nursing, adult education and patient education. Must possess organization and communication skillls. EMR or billing system exposure at the practice level. Must possess the ability to develop and present information/educational offerings to individuals and group audiences as appropriate (healthcare providers, patients/family members, general public, etc.).
Two years' experience in Diabetes Education preferred.
Diabetes Self-Management Education (DSME) preferred.
Licenses, Registrations, or Certifications
Current TX Nursing License; Certified Diabetes Educator preferred.
