Registered Nurse Care Manager - Case Management Job in Corpus Christi

Summary:

RN Case Manager will work with physicians and multidisciplinary team members to develop a plan of care for each assigned patient from admission through discharge. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. Assesses and responds to patient/family needs by coordinating efforts of other team members. Identifies and resolves barriers that hinder effective patient care.

Located just minutes from the shores of Padre Island, CHRISTUS Spohn Hospital Corpus Christi-South provides surgical and medical services for our patients in south Corpus Christi. Perfectly positioned to serve the rapidly-growing south side of Corpus Christi, CHRISTUS Spohn Hospital Corpus Christi - South is licensed for 158 beds. South offers a full range of medical and surgical services including pediatrics, obstetrics, skilled nursing, emergency, intensive care as well as advanced diagnostic and imaging. The hospital is also home to a Level III Neonatal Intensive Care Unit which enables recovering mothers to remain close to their premature or special needs babies.

Requirements:

• Graduate of an accredited school of nursing.
• Minimum of 3 years of clinical experience required. Case management, utilization management quality management or discharge planning experience preferred. Knowledge of Interqual criteria preferred.
  
• Current Texas State RN License.
